I had the same problem with my 05 rav4 and replaced my tensioner and turned out to be the same thing. ac was loose.

Ive been working on cars since i was 11,i just turned 53, believe me, we have all done stupid stuff not meaning to,i remember when i was a kid growing up in a farm,i didn't have a drivers license yet so my dad and i found a 1957 land cruiser in a old wrecking yard and he asked me if i wanted it? really? hell yeah!!! so it was my "farm car", it needed lots of work and it was in rough shape so i cut my teeth on that thing, 4 speed monster.
First time i changed plugs on it i pulled the wires off,label wires? what for? once i got done changing the plugs i just put the plug wires where they looked cool,once i fired that thing up it was bucking like a 3 second bronco, my old man asked me about he plug wires and of course the answer was uhh?? i learned a lesson that day, my dad explained to me about timing and why certain things need to be in certain places etc
Another time me and my buddy in our teens where working on his car on the front suspension,he had one of those 20 dollar floor jacks,we cranked that thing up and he actually sat under the front spindle while he was working on the brakes (no jackstands of course) , his legs were right below the car,i was right behind him and all of the sudden i started hearing this creaking sound and i grabbed him by the neck ( i guess Adrenalin) and just yanked him right out as that car hit the ground, he couldn't turn his head around for a month but he was very grateful,lol, yep we have all done stupid stuff before.

Glad your OK. I don"t even want to tell you what I did. It was worse than what you did, I'm embassed to think that I could have been so stupid and I could have been killed. It is humbling and a reminder of how fast things can happen. It is always in the back of my mind now working alone in the shop. If money can fix it, it's not a problem. Had my share of these. I did learn never to leave the driver side window up. Made a point to let in down before getting out. Probably needs rear brakes. I learned to check in a safe place they hold to 1500 rpm in gear. Remember a Advisor jumped in a car in my bay left the door open and reverse he went, trying to keep from paying me diagnostic charge. Folded the door back. Funny thing is there was no diagnosis charge. The man just bought it. They have to take it back and get him another one.
Wes when lifting the 3.0 liter on a 2012 Cadillac CTS to replace the Engine mounts. Please watch the intake to windshield cowl for interference or like me you’ll be replacing the Windshield on your dime. Please be careful. You to valuable to lose.
Wes, I can relate. I have my own channel now and prior to my making a video called "Bowl of Death" I had an act of stupidity nearly put my lights out. The piece on my lathe exploded, took me right between the eyes just seconds after I had taken my face shield off. That was the day I learned you can never be safe enough. I used to tell the people working for and with me that "Nobody gets hurt on my watch". Except me I guess… Great Video!

It happened because you become complacent, you been a tech for a while you don't pay as much attention as you should ,2 years ago I re geared a jeep gladiator I had it running in the air in 4 low I hear a noise by the rear differential my dumbass goes to touch by the rear pinion the fucker grabbed my finger and twisted it off like a bottle cap
